anyone else having ghost signals issues with inpulse 500?

i got an inpulse 500 and 3-4 months after it started sending ghost signals. i contacted the warranty, they got the controller and sent a new one. then about 5 months after it started again with the same issue. anyone else had this problem?

ps.: i have the drivers updated, updated os, tested with serato, djuced and virtualdj, with 2 different cables in 4 usb ports.
